ASSESSMENT
 
The assessment checklist lists the different areas we need to assess in order to acertain the strengths and 
needs of the child and guide us for further intervention.  
Observe the child's behaviour and answer the following questions on the checklist as yes or no and also note in detail, how  the child responds.  
The childs strengths will be those areas with the answers being YES and the childs needs will be those areas with the answers being No.

Does child have a way of telling you what he wants?
Does child have a way of telling you what he does not want?
Does child recognise familiar people?
Does child take turns with you- smiling, laughing, making sounds, playing?  
 
HEARING/LISTENING Does child respond to loud sounds?
Does child respond to quiet sounds?
Does child look to where sounds are coming from?
Does child recognise familiar voices?
Does child listen carefully to voices?  
 
VISION/LOOKING Does child look at objects?
Does child follow objects?
What does child like to look at most?
Is child interested in what's going on around him?  
 
TOUCH Does child respond when you touch him?
Does child respond differently to different types of touch,e.g., stroking, patting, rubbing, tickling?
Does child hold objects?
Does child feel objects?
Does child explore and play with objects with his hands?  
 
SMELL Are there smells that the child likes?
Are there smells that the child does not like?
Does child know it's time to eat when he smells food?  
 
TASTE Are there tastes that the child likes?
Are there tastes that the child does not like?
Does child eat foods of a variety of textures?  
 
MOVEMENT Does child like being moved?
Does child like vigorous movement?
Does child like gentle slow movement?
Does child like to be swung?
Does child enjoy moving his body?  
 
AWARENESS OF HIDDEN OBJECTS Does child show interest in people and objects?
Does child notice when an object is hidden from him?
Does child search for an object when it is half hidden?
Does child search for an object when it is completly hidden? 
 
ACTING PURPOSEFULLY Does child hold objects?
Does child reach out for objects?
Does child know that if he makes a certain sound or action that a particular thing will happen,e.g.:   
- if he makes sound, his mother will come to him   
- if he reached out his arms, he will be lifted up.   
- if he pulls a string the toy attached to it will move   
- if he reaches for an object someone will give it to him.  
 
COPYING ACTIONS If you copy one of the child's action does he copy you?
Does child copy new actions that you make?
Does child copy other people's actions spontaneously?  
 
MAKING SOUNDS Does child make sounds?
Does child make a variety of sounds?
Does child  babble?
Does child copy sounds made by adult?
Does child make more sounds at certain times?
Does child make sounds to get things done to him?
